Applications for registration

(1) All applicants shall submit to the Registrar a duly completed and signed application formForm I. Schedule. (a) as set out in Form I of the Schedule hereto, together with a non-refundable fee of twenty-five dollars, in respect of applications for a day care licence for new facilities coming into operation after the commencement of these Regulations; (b) as set out in Form II of the Schedule hereto, together with a non-refundable fee of twenty-five dollars, in respect of applications for a temporary day care licence for existing facilities which were in operation before the commencement of these Regulations; Form II. Schedule. (c) as set out in Form III of the Schedule hereto, together with a non-refundable fee of fifteen dollars, in respect of renewal applications for day care facilities already licensed under these Regulations. Form III. Schedule. 10 Social Service Agencies (2) An application made under subregulation (1) above shall be submitted to the Registrar– (a) at least sixty calendar days before the proposed date of commencement of operations, in the case of new day care facilities whose operations shall commence after the entry into force of these Regulations; (b) at least sixty calendar days after the commencement of these Regulations, in the case of day care facilities already in existence before the commencement of these Regulations; and (c) at least sixty calendar days before the expiration of a current licence, in the case of an application for the renewal of a licence. Schedule. (3) The application forms set out in the Schedule hereto as Forms I to III shall be available to applicants at any office of the Ministry every Monday to Friday between the hours of 8 o’clock and 12 o’clock in the forenoon and 1 o’clock and 5 o’clock in the afternoon.
