Claim to be made in writing on appropriate form

(1) Every claim for a pension under these Regulations shall be made in writing to the Committee on the form prescribed in the First Schedule to these Regulations, unless the manager, in his discretion, waives this requirement. (2) Claim forms shall be supplied free of charge by the
