Form for application for extension

An application for extension of the permitted hours pursuant to any Order made under section 4 of the Clubs (Registration) Act shall be in the form as set out as Form 1 of the Schedule hereto, and shall be made to the Commissioner of Police at least two clear days before the date to which such application refers.
