Election of spouse to waive death benefit, survivor pension

(1) For the purposes of section 26 of the Act, a spouse may consent and waive his or her right to a death benefit from a pension plan in the form set out as Form 5 of Schedule I. CAP. 30:01. (2) For the purposes of section 27 of the Act, a spouse may consent and waive his or her right to a survivor pension from the pension plan in the form set out as Form 6 of Schedule I. Form 6.
