Lost certificate

Where a certificate has been lost, stolen, destroyed, mutilated or defaced, the licensee may issue to the unit holder a new certificate in replacement thereof but the applicant shall furnish to the licensee satisfactory evidence of the loss, theft, destruction, mutilation or defacement of the original unit certificate and such form of indemnity as the licensee may require.
