Form required

(1) In submitting a document to the Office for any purpose, the person submitting the document must use the form set out in Schedule II that is appropriate for the purpose, but the form may be modified in such manner as is necessary in the circumstances. Schedule II. (2) In submitting a document to the Office, one copy only of the document need be submitted.
