Application for certificate of competency

An application for any certificate of competency shall be made to the Ports Commissioner using the form set out in Schedule I and shall be accompanied bySchedule I. (a) a police record; (b) a medical certificate, certified within six months of application, attesting to the health of the applicant including vision, hearing, and mental capacity; 80 (c) the appropriate fees prescribed in Schedule II; and Schedule II. (d) proof of address in the manner specified by the Ports Commissioner.
