Applications for licences

(1) A person desirous of becoming a certified user shall apply to the Board for a licence, on the Form prescribed in Schedule I hereto. (2) The person making the application under subregulation (1) above, shall send the application to the Board, together with the fee specified in regulation 6, below. Schedule I. (3) The Form prescribed in Schedule I hereto shall be used for initial applications for a licence, or for applications for the renewal of a licence. Schedule II.
