Form of Application for Belize Passport

(1) An applicant shall submit his application for a passport to be issuedSchedule 1. Form 1. (a) in his own name, in the form set out as Form 1 in Schedule 1; or Schedule 1. Form 2. (b) in respect of a child, in the form set out as Form 2 in Schedule 1, together with the particulars required under regulations 6 and 7 and upon payment of the relevant fee prescribed in regulation 8. (2) An applicant may only apply for a passport in respect of a child if the applicant is– (a) a parent of that child; or (b) a relative or guardian of that child. (3) An application made by a relative or guardian of a child pursuant to subregulation (2)(b), shall be supported in writing by an affidavit signed and duly witnessed stating that the applicant has sole custody of that child and the period of time that the applicant has had custody of that child.
