Discharge certificate

The particulars to be contained in a certificate of discharge shall be– (a) number; (b) name, including Christian or forenames; (c) date and place of enlistment; (d) physical description of soldier on leaving regular service; (e) rank of soldier on leaving regular service; (f) assessment of conduct and character on leaving regular service with the signature of the officer making the assessment; (g) date of discharge; (h) reason for discharge; (i) total regular service on discharge; (j) signature of issuing officer.
